According to Beating monitoring, OpenAI’s core product负责人 Tibo responded to the GPT-5.6 incident in which files were mistakenly deleted, saying that the team has investigated several related reports. The most common cause is that the model incorrectly sets the temporary directory to $HOME, then accidentally deletes the user’s home directory instead of the temporary directory. OpenAI stated that incidents of this kind mainly occur in Full Access mode, and that Codex did not have sandboxing enabled and automated review enabled. The team will update developer prompts, guide users to use safer permission modes, and add more security protections.
